ICE Medical Information Card

Professionally printed I.C.E medical cards, which can be customised. The cards are digitally printed on both sides. The card is the same size as your debit or credit card.

Once you have placed an order, please message us via eBay with the following information:

Medical Condition
Name
Date of Birth
Address
Emergency Contacts, Name, Relationship, Mobile Number and Home Number up to 2 contacts.
Blood Type – if known
Allergies (nuts. Penicillin)

All fields are optional but recommended.

Any fields not provided will be left blank or removed from the template. Once your card has been printed we do not keep a hold of your data.

Please ensure that all data provided is correct before sending it over to us as once printed we will not be help responsible for any errors or inaccuracies.
